By filling and submitting a request for service you are accepting & agreeing to all of ICEPROS, LLC., RATES / TERMS / POLICIES (Click on menu on to view Company Policies Rates & Terms) --- An answer to every question is required otherwise the system will not allow submittal of your request, so if any question is unknown inputting "N/A" is acceptable. --- Payment for requested services including initial travel charges to location to estimate repairs are due upon completion of service and Method of Payment must be specified in advance, regardless of current warranty status. --- Open accounts must be established in advance and account must be in good standing.
